Abstract
Diabetes mellitus is a chronic metabolic disease that requires long-term management, with medication adherence being a critical pillar of effective therapy. Poor adherence to prescribed antidiabetic medications contributes significantly to uncontrolled blood glucose levels and increased risk of complications. This community service program aimed to improve patient knowledge and medication adherence through innovative diabetes medication counseling at Kelurahan Sei Agul, Kecamatan Medan Barat, Medan. The activity was conducted on Saturday, March 13, 2025, involving 35 participants comprising diabetes patients and community members. Methods included health counseling, interactive discussion, Pill Box simulation workshops, digital literacy training for medication reminders, and teach-back evaluation. The innovative approach incorporated storytelling, myth-versus-fact discussion, the 5-Rights of medication principle, and practical tools such as medication cards and weekly compliance checklists. Results demonstrated improvement in participants' understanding of proper medication use, including correct dosage, timing, and administration; recognition of hypoglycemia symptoms; and heightened awareness of the importance of medication adherence to prevent diabetes complications. Nurse-led educational interventions through community service are effective strategies for increasing medication adherence and supporting optimal diabetes management
